Horizon Award

The Horizon Award is the most prestigious recognition bestowed on an alum by the Alumni Association. Alums may receive the Horizon Award only once in their lifetime.

Nomination Process

Please include your name, address, phone and email address.

Submit a nomination letter that is no more than two pages. In your submission, include examples and details as to why your candidate should be considered for the Horizon Award. Suggested topics to address in the nomination letter include:

  • Outstanding contributions to her/his field.
  • Dedication to healthcare.
  • Demonstration of high professional standards.
  • Consistently demonstrates exceptional personal integrity and character and is a superior role model to peers, students, and members of the Alumni Association.
  • Exemplifies the Alumni Association's core values of Caring, Excellence, Holism, Learning & Respect.

The recipient(s) will be notified and invited to attend the annual Reunion Dinner to receive the award.

Horizon Award Areas of Excellence

Clinical Excellence 

Explain, including examples, how your candidate displays some or all of the following:

  • Demonstrates clinical expertise and outstanding achievement in her/his chosen career or area of practice.
  • Has received recognition for exemplary professional practice.
  • Demonstrates a breadth of knowledge in her/his area of practice.
  • Develops creative approaches to her/his practice that contribute to the holistic care of the client as well as family members.
  • Demonstrates leadership.
  • Serves as a mentor/preceptor that inspires peers' professional practice.
  • Enhances the image of the profession through her/his practice.
  • Advances the scope and practice of her/his profession.
  • Participates in community affairs, legislation, or organizations that affect professional practice.

Volunteer Excellence

Explain, including examples, how your candidate displays some or all of the following:

  • Freely gives her/his talent, time and efforts to an organization or group without financial gain.
  • Demonstrates outstanding volunteer efforts in her/his community, including religious, civic and/or charitable organizations or programs, either local or national in scope and structure.
  • Distinguishes herself/himself for contributions which enhance the wellbeing of the community.
  • Demonstrates a passion for and dedication to a cause.
  • Committed to excellence, no matter what the volunteer activity.
  • Exemplifies the best of volunteerism, a sense of caring and responsibility for others that connects citizens and solves community problems.
  • Has received recognition for exemplary volunteer efforts.
  • Demonstrates exemplary leadership, creativity, cooperation and hard work.
  • Changes lives as a result of her/his volunteer efforts.
  • Serves as a model and inspiration to other volunteers.

Educational Excellence

Explain, including examples, how your candidate displays some or all of the following:

  • Facilitates change and growth resulting in positive impact on healthcare delivery.
  • Participates in research, publishes or makes presentations on healthcare topics.
  • Has made contributions to education at the local, state, national, or international level.
  • Participates in community affairs, legislation, or organizations that affect professional practice.
  • Promotes the development, dissemination and utilization of healthcare education.
  • Inspires others through service as a mentor/preceptor.
  • Demonstrates excellence in teaching or promoting education.
  • Influences professional practice and the public's image of healthcare through excellence in teaching.
  • Significantly influences healthcare education by creating innovative and holistic approaches.
  • Has influenced the present and future healthcare practice through education.
  • Is recognized by students and alumni for excellence and compassion as a teacher.
